Your Valid Passport
You must send your valid and signed passport with at least 6 months remaining validity from your planned date of entry and at least one blank visa page.

Two Completed Visa Application Forms

The two visa Application Forms must be completed fully and signed.

Two Passport Photos
You must provide two passport photographs that meet Passport Photograph Regulations.

Company Letter

1. You must provide a typewritten letter with your company letterhead, signed by a company officer and addressed to:

   The Embassy of Peru
   Visa Section, Washington DC:

The letter should contain the following:

  • Nature of business to be conducted in Peru.
  • Name and address of your business contact in Peru.
  • Guarantee of financial responsibility and of return transportation for the visa applicant.

2. Or an invitation letter from the host company in Peru.

Proof of Legal Status in the US (If applicable)
For non-US citizens, you must provide proof of your resident of legal status in the USA. (Examples: copy of green card, valid US visa and I-20, or valid I-AP6
